Instructor Led Training Resources

Below you will find current versions of the required Instructor Led Training (ILT) forms and resources. Please download each of the following and print as many copies as necessary before your classroom session begins.

Sign-in Sheet

This sign-in sheet must be used when teaching any Instructor Led Training course. Select the course you will be teaching from the drop-down menu, fill in the blanks, and print. All students attending the training are required to sign in. Credit for completion will be determined based on sign-in sheets, so each attendee must fill in their information completely, accurately, and legibly to receive credit. Completed sign-in sheets should be scanned and emailed to onlinecourses@iatsetrainingtrust.org.

Acknowledgment & Release Agreement

Review the following acknowledgment and release language at the start of each class. Please feel free to make copies of this acknowledgment document to hand out to students as you are reviewing the language with the class.

Please note there are two versions of this document. Use option #2 for the Pandemic Safety course. Use option #1 for all other courses.

This booklet contains additional resources that you may use and choose to share with the class. These resources can be used at the instructor’s discretion and are not required.

Emergency Action Plan

We strongly encourage local unions to develop an Emergency Action Plan (EAP) for every training session, meeting, or event to keep people safe. The EAP describes what actions need to be taken to ensure the safety of those attending in the event of an emergency. The EAP includes floor plans or maps that indicate the escape routes for the venue, the exits, remain-in-place locations, and fire extinguishers. Many venues already have their own EAP, so you may not need to develop your own. It will only take a few minutes to present your EAP to the participants before training starts.

Evaluation Form

You must use this evaluation form when teaching any Instructor Led Training course. Select the course you will be teaching from the drop-down menu, fill in the blanks, and print enough copies for your students. All students attending the training should be asked to complete this evaluation form. Completed surveys must be scanned and emailed to onlinecourses@iatsetrainingtrust.org.
